U.S. Passport Day is back! Join us on Saturday, June 12, 2021, from 8 AM – 2 PM at Eastvale City Hall! Walk-ins will be accepted, no scheduled appointment is needed! ⠀

Please be sure to bring the following:

• A completed, but unsigned application form. Forms are available at travel.state.gov.
• Proof of identity such as a driver’s license
• Proof of U.S. citizenship such as a birth certificate (original copy)
• Appropriate forms of payment: personal check, money order, or cashiers check⠀
