The Human Resources & Risk Management Department processes personal injury, property or vehicle damage claims filed against the City of Eastvale.  The terms or acceptance of claim forms by the City is not an admission of any fault or liability on the part of the City or any office, agent, or employee thereof.

Submit a completed  form in person or by mail to:

Please note that City Hall is open Monday through Thursday, between 7:30 a.m. and 5:30 p.m.

The claim will then be submitted to Human Resources & Risk Management for processing. This can take up to 45 days to accept or reject the claim. Please be sure to include any quotes, receipts, photos, or other documents to support your claim.
