Form 700 - Statement of Economic Interests
City Council Members are elected officials in Government Code Section 87200 and are required to file Statement of Economic Interest Forms with the City Clerk's Office.

Form 802 - Tickets and Passes Distribution
The Policy provides the following:
1. The distribution of the tickets or passes to, or at the behest of, an Eastvale Official must accomplish a governmental and/or public purpose. The following list of government and/or public purposes the City may accomplish through the distribution of tickets is illustrative rather than exhaustive:
a. Facilitating public recognition, support, or award-giving on behalf of the City at an event.
b. Facilitating the attendance of an Eastvale Official at an event where the job duties of the official require his or her attendance at the event.
c. Promotion of intergovernmental relations and/or cooperation and coordination of resources with other governmental agencies, including, but not limited to, attendance at an event with or by elected or appointed public officials from other jurisdictions, their staff members, and their guests.
d. Promotion of Eastvale resources and/or facilities available to the public.
e. Promotion of City-run, sponsored, or supported community programs or events.
f. Promotion, supporting and/or showing appreciation for programs or services rendered by charitable and non-profit organizations benefiting the public.
Form 806 - Public Official Appointments
An official may vote to hold another position even if the voting official will receive additional income so long as the public is informed prior to the vote or consent item. FPPC Regulation 18702.5 provides for the filing of a Form 806 under these circumstances. Each agency must post on its website a single Form 806 which lists all of the paid appointed positions to which an official will vote to appoint themselves. When there is a change in compensation or a new appointment, the Form 806 is updated to reflect the change. The form must be updated promptly as changes occur.
